1. Electronic inquiries to the healthcare institution “4th City Clinical Hospital” can be submitted by placing the inquiry in a special section on the official website of the global computer network “Internet” (see the Electronic Inquires section). Electronic inquiries sent to the institution in some other way will not be considered.
  2. Electronic inquiries must comply with the requirements established by clause 1, paragraphs two to four of clause 2 or paragraphs two to five of clause 3 of Article 12 of this Law of the Republic of Belarus No. 300-3 as of July 18, 2011 “On inquiries of citizens and legal entities” (hereinafter referred to as the Law), and also contain the email address of the applicant.
  3. Electronic inquiries are stated in Belarusian or Russian. Electronic inquiries submitted by representatives of applicants must be accompanied by electronic copies of documents confirming their authority. If these requirements are not complied with, an electronic inquiry may be left without consideration on the merits in the manner established by clause 4 of Article 15 of the Law.
  4. If necessary, additional documents and (or) information (documents confirming the authority of the applicants' representatives, documents on the results of the previous consideration of applications and other documents and (or) information necessary to resolve the issues stated in the applications) may be attached to the electronic application. The acceptable formats for attached documents and (or) the information specified in paragraph four of part one of this clause in electronic form and their graphic images on paper (scans) are Portable Document Format/A (PDF/A), Office Open XML (DOCX), Dual Markup (DOC), Rich Text Format (RTF), Text File (TXT), Open Document Format (ODT), Data Archiving and Compression (ZIP, RAR), Portable Network Graphics (PNG), Tagged Image File Format (TIFF), Joint Photograph Experts Group (JPEG), Joint Photograph Group (JPG).
  5. Electronic inquiries may be left without consideration on the merits if:
  • inquiries do not meet the requirements established by the norms of the Law;
  • are subject to consideration in accordance with the legislation on constitutional legal proceedings, civil, civil procedural, economic procedural, criminal procedure legislation, legislation determining the administrative procedure, inquiries are an employee’s inquiries to the employer or a different procedure is established in accordance with legislative acts filing and consideration of such inquiries;
  • inquiries contain questions the solution of which is beyond the competence of the organization they have been sent to, are not related to the quality of goods produced (sold) by them, work performed, services provided;
  • the deadline for submitting a complaint has been missed;
  • the applicant has submitted a second inquiry, including the one entered in the customer comment book, and it does not contain new circumstances relevant to the applicant's termination of correspondence on the issues set forth in the inquiry.

Electronic inquiries of citizens, including individual entrepreneurs (hereinafter referred to as citizens), and legal entities are considered by the healthcare institution “4th Clinical Hospital” in accordance with the requirements of the Law of the Republic of Belarus No. 300-3 as of July 18, 2011 ”On inquiries of Citizens and Legal persons” (hereinafter referred to as the Law)
  2. The withdrawal of an electronic inquiry is carried out by submitting a written application or by sending an application in electronic form in the same way that the electronic inquiry was sent.
  3. If, in order to consider an electronic application, on the merits, it is necessary to indicate the personal data of the applicant or other persons, with the exception of those contained in the application, the applicant is invited to apply either orally or in writing.
  4. Responses (notifications) to electronic inquiries are sent to the applicants' email address indicated in the electronic inquiries. Written answers are given to electronic inquiries (written notifications are sent) in cases where:
  • the applicant in his electronic inquiry asks to send a written response or to send both a written response and a response to his email address;
  • a response (notification) has not been delivered to the email address indicated in the  electronic inquiry due to technical problems.

  2. If more than ten incoming electronic inquiries of similar content from different applicants are received, answers to such applications may be posted on the official website of the institution without replies (notifications) sent to the applicants.
